Calling All Books
Good, like-new books for PK – 6th grade – that students would enjoy receiving the last week of school when the library stops checking out books for the summer. Our goal is to have enough for each child take 1-2 books home to keep and remind them the library will be open this summer: Wednesdays from 5-7pm June 12, June 26, July 10, July 24 & August 7th (supper included – all free – watch for upcoming details!). Please send like-new books with your child to the school library; thank you for your donations.

Check out the Crestview Library Home Page on Destiny, the district’s web based automated library system. Parents and students can use any computer with an Internet access to view the Crestview library catalog. You’ll also find links to some excellent subscription data bases - free to WDM families. 2012-13 Emergency Dismissal Plan - Please complete a form for EACH student. This document will be kept with classroom teachers to assure us that all students have a plan to arrive home safely when school is dismissed early due to an emergency situation including but not limited to:
- Hazardous weather conditions, Community saftety concerns or Loss of water or power at our school for an extended time.
2012-13 School Volunteer Application Form - This is to be completed by ANYONE who volunteers to work with students.
2012-13 Volunteer Driver and Liability Form - This document must be completed by ANYONE who uses their own vehicle to transport students for school trips. When returning the form to school, please remember to include a copy of your current driver's license and automobile liability insurance card.
2012-13 Release of Information Form - This document is to be completed if you do not wish to have your child's photo released for educational purposes. The West Des Moines Community School District often uses student photographs and/or names in school/district publications and websites. : no names are included with webpublished photographs. In addition, no personal information such as address/phone number/photograph is included with web-published names. If you approve the district using photographs and/or your child's name in publications and onwebsites, you do not need to return this form.
